Just go your local courthouse, ask where you can go to remove one your daughters middle names, it should be quite easy. I had my name respelled once and they did right there didn't have to file a petition or anything. After that is done just take the new birth certificate to the SS office and make the changes there and anywhere else you need to make the change.
Reply:Changing a persons name usually requires paperwork to be done by the courts in most states. I would recommend that the best way to get things started would be to contact a attorney in your local area. Most areas always have some attorney that will talk to you with out a consultation fee or charge just a small office fee to discuss matters like this. They can explain what would have to be done. Most larger cities also have a public legal based service for those who have limited income which provides for free legal help. Sometimes the Clerk of Court can point you in the right direction, but most will shy away from giving legal advise , but they can provide you any required forms.
Reply:You have to go to the courts and fill out a name change form. Then if the judge approves, you go to the Social Security office and get it changed.
Reply:First of all, i would gather up all my daughter's information. Then, i would go to the courthouse building, to the department of vital statistics and let them know your child has two middle names, and you'd like to drop one of them. it may cost you a few bucks, but eventually it'll pay off in the long run. They should have a website posted on the internet, or just run to your courthouse bldg!!!
Reply:call or go to the probate court in your town.
Reply:you just change it at the social security office, like you do when you get married.
